How do you format a book review?

I think I understand what needs to go in a book review. Like, you write about the plot and main points of the book and then input your opinion. However, I don't know how to go about formatting it. Do you make each one of those a separate paragraph or do you intertwine it all into the same paragraph and just make the review one large paragraph?
